PROMOTING GOOD HEALTH
Our goal is to promote good health for our students and staff. As the districts nurses we promote good health by performing a variety of duties. The duties include assessing student health needs, maintenance of student health and immunization records, monitoring attendance, providing first aid to students and staff, conducting mandatory health screenings, which include hearing and vision testing and providing health education.
NEW STUDENTS
If your family is moving into the state of Illinois, all incoming children are required to have a physical completed within 30 days of enrollment.The physical needs to be on the appropriate form. The form is titled "State of Illinois Department of Human Services Certificate of Child Health Examination".
PHYSICAL EXAMINATION
Physical Examinations are required for children entering Early Childhood, Kindergarten, 6th grade and 9th grade.
VISION EXAMINATION
Vision Examinations are required for all children in Kindergarten. This exam needs to be completed and on file by October 15th. Vision Exam Forms are available online as well as a Waiver form.
DENTAL EXAMINATION
Dental Examinations are required for Kindergarten, 2nd and 6th grades This exam needs to be completed and on file by May 15th. Dental Exam Forms are available online as well as a Waiver Form.
